PLEASE NOTE:

• Each person for whom you are requesting information must sign a Disclosure & Release. Please do this before filling out this form. You are required to attach each person’s D&R to this form via the provided upload feature before submission.
• If you deny credit to ANYONE, you must provide them with a Notice of Adverse Action. This will allow them to order a copy of their credit reports from the reporting agencies for free.
