How to Order Business Credit Reports

Registration is mandatory.

STEP 1

On the FASTFAX Order Form, provide information about yourself. We need to know:

• Your name
• Your company or your last name (name of account)
• Your Authorization Code # (unless account is new)
• Your phone #
• If you leave your Fax # your report will be returned by fax
• Any other special instruction

STEP 2

Target company information:

• Name of Company to check
• Address of company, including zip
• Identifying factor: Federal Tax ID or Main Phone #.

STEP 3

Fax to 800-522-8437.

Your report should be returned to you within 90 minutes.
